From time to time, students are required to take medication in order to help them to stay safe and healthy. If your student is in need of taking medication while at school, please complete the corresponding form from the options found below. If you are unsure about which form to complete, questions can be submitted to the Elementary/JH School Office. Our staff will be happy to help guide you through the completion process by phone at (620) 723-2332.
